1.3Page 23, after line 35, insert:

1.4    "Sec. 17. Minnesota Statutes 2006, section 144.2215, subdivision 1, is amended to read:
1.5    Subdivision 1. Establishment. Within the limits of available appropriations, the
1.6commissioner of health shall establish and maintain an information system containing data
1.7on the cause, treatment, prevention, and cure of major birth defects. The commissioner
1.8shall consult with representatives and experts in epidemiology, medicine, insurance,
1.9health maintenance organizations, genetics, consumers, and voluntary organizations in
1.10developing the system and may phase in the implementation of the system. After the
1.11parents have provided informed consent under section 144.2216, subdivision 4, the
1.12commissioner shall offer the parents with their informed consent a visit by a trained health
1.13care worker to interview the parents about:
1.14    (1) all previous home addresses, occupations, and places of work including from
1.15childhood;
1.16    (2) the time and place of any military service ; and
1.17    (3) known occasions or sites of toxic exposures."
1.18Renumber the sections in sequence and correct the internal references
1.19Amend the title accordingly
