1.3    "Sec. 4. Minnesota Statutes 2008, section 256B.0751, subdivision 7, is amended to

1.5    Subd. 7. Outreach. Beginning July 1, 2009, the commissioner shall encourage
1.6require state health care program enrollees who have a complex or chronic condition to
1.7select a primary care clinic with clinicians who have been certified as health care homes,
1.8if there are two or more primary care clinics with clinicians who have been certified as
1.9health care homes available to the enrollee.

1.10    Sec. 5. Minnesota Statutes 2008, section 256L.05, subdivision 4, is amended to read:
1.11    Subd. 4. Application processing. (a) The commissioner of human services shall
1.12determine an applicant's eligibility for MinnesotaCare no more than 30 days from the
1.13date that the application is received by the Department of Human Services. Beginning
1.14January 1, 2000, this requirement also applies to local county human services agencies
1.15that determine eligibility for MinnesotaCare.
1.16(b) Upon receiving an application, the commissioner or local county human services
1.17agency shall assign one individual as the coordinator of the application. The coordinator
1.18shall be responsible for all communications with the applicant throughout the application
1.19process and upon renewal.

1.20    Sec. 6. FEDERAL APPROVAL.
1.21The commissioner of human services shall seek federal approval, if necessary, to
1.22implement Minnesota Statutes, section 256B.0751, subdivision 7."
